Terms of Use and Conditions eCVI Account

• It is my responsibility to properly use my eCVI account and approved digital signature.
• It is my responsibility to take reasonable care to protect, secure and prevent the misuse of my TAHC eCVI account, password and approved digital signature.
• It is my responsibility to immediately report to the commission the loss, theft, or deliberate or accidental misuse of my eCVI account, password or approved digital signature.
• I understand that for data security reasons I will be required to change my password every 60 days.
• It is my responsibility and I agree to issue eCVIs in my name only and in accordance with any applicable standards, regulations or laws of the Texas Animal Health Commission (commission), Texas State Board of Veterinary Medical Examiners and United State Department of Agriculture.
• I understand that an eCVI is a governmental record and that making a false entry or alteration to an eCVI is a criminal violation pursuant to Texas Penal Code Section 37.10.
• I understand and agree that an eCVI must be accurate and fully completed before it is issued.
• I understand that it is my responsibility to maintain current contact information.
• I understand that eCVIs must be purchased one at a time (no bulk purchases) and that payment must be made at the time the eCVI is ordered.
• I understand that eCVIs must be purchased and completed in a single browser session. It is not possible to "save" a partially completed eCVI.
• I understand the fee for each eCVI is $7 and that payment can only be made online by credit card at Texas.gov. I understand that Texas.gov will charge a payment processing fee that is in addition to the $7 eCVI fee. (Note: Your payment information will not be stored by the commission or Texas.gov.)
• I understand that eCVI credit card payments require entry of the card CVV (sometimes referred to as CVC or security code) and the zip code of the billing address associated with the card.
• I understand that upon completion of the form, the eCVI will automatically be emailed to the commission, the destination state's animal health officials, and the issuing veterinarian. Emails will be sent from no-reply@tahc.texas.gov; please check your email preferences to ensure eCVI emails don't end up in your Junk/Spam folder.
  (Note: The completed eCVI will also display as a PDF and may be saved to a computer and/or printed.)
